All insurance providers require signed consent for the Clinic to submit claims and receive payments on your behalf. Please allow time to complete forms prior to your appointment time so that we can submit during your treatment. Depending on plan and coverage, all or a portion of the cost may be covered. Some plans will pay the Clinic directly; others pay only the insured but allow us to submit on your behalf.
If the provider requires further processing, we file the claim on your behalf; the payment go to you directly. You, then, are responsible for payment to the Clinic on the day the service is provided.
We have learned, through trial and error, that most providers do not accept electronically submitted claims for Secondary Coverage. These claims usually must be submitted manually. In these cases, the Clinic must be paid directly.
